Duties:
- Provide personalised care for patients with diabetes, focusing on prevention, treatment, and ongoing management.
- Assess and monitor patients’ health status, develop care plans, and offer tailored interventions.
- Educate patients and their families on diabetes management, including lifestyle changes, diet, medication, and blood sugar monitoring.
- Collaborate with the multidisciplinary healthcare team, including doctors, nurses, dietitians, and other specialists to ensure holistic care.
- Stay updated on the latest advancements in diabetes care, treatments, and technologies.
- Offer emotional support and guidance to patients, promoting overall well-being and self-management.
- Maintain accurate patient records and documentation in compliance with healthcare regulations.
- Participate in ongoing professional development and training opportunities
Requirements:
- Bachelor’s degree in Nursing
- Relevant certification or qualification in Diabetes Care (e.g., Certified Diabetes Educator, or equivalent).
- At least 1 year of experience working with diabetes patients.
- Strong knowledge of diabetes treatment protocols, medications, and the latest research in diabetes management.
- Excellent communication and interpersonal skills.
- Compassionate, patient-centered approach to care.
- Ability to work both independently and as part of a collaborative team.
